1. Preparation of Evaluation Plan
a. Read and analyze approved MSAP application.
b. Edit and/or create revised evaluation plan.
c. Develop a data collection plan.
d. Create and distribute data collection templates.

2. Meet with MSAP project management to review evaluation documents and plan for evaluation process.
a. Make revisions and send completed documents to MSAP project management.

3. Evaluation Team visits (fall) all funded schools
a. Prepare a summary of the MSAP site visit.
b. Provide feedback (commendations and recommendations) on the activities.
c. Send a written report to MSAP project management.

4. Evaluation Team visits (winter) all schools
a. Prepare a summary of the MSAP site visit.
b. Provide feedback (commendations and recommendations) on the activities.
c. Send a written report to MSAP project management.

5. Evaluation Team visits (spring) all funded schools
a. Prepare a summary of the MSAP site visit.
b. Provide feedback (commendations and recommendations) on the activities.
c. Send a written report to MSAP project management.

6. Classroom Observations (at least once a year)
a. Beginning in project Year 2, observe in classrooms at MSAP schools that are implementing
instructional techniques and/or using content learned in MSAP trainings in their classrooms.
b. Use an observation rubric to provide written feedback to project management.

7. Zoomerang Surveys for MSAP teachers, students, and parents (groups as needed)
a. Develop surveys and collaboration with MSAP project management to revise/edit surveys.
b. Post surveys to Zoomerang (or other) website.
c. Notify project management of web link for survey.
d. When surveys are completed – close surveys
e. Analyze and report survey data.

8. Annual Performance Report (APR)
a. Develop a template for the USDE-required APR—using the ED-524b format.
b. Complete an ED-524b chart for each project objective and/or performance measure.
c. Prepare an Executive Summary for the APR.
d. Send an electronic file containing the completed APR and a notebook with the printed report
to project management.
e. At the end of the third year, prepare a final evaluation report. This document is due within 90 days of the end of the project.

9. Provide guidance and feedback
a. In meetings with staff provide guidance and suggestions on project implementation and improvements
b. Provide written formative evaluation reports following each site visit.
c. Develop a rubric for use when visiting MSAP participants’ classrooms and provide feedback for each
observation – as well as an overview of the process.
d. Following each site visit – provide written summaries of the visit and/or focus group meetings.
Send this feedback to project management.
e. As needed, meet with project management or other district representatives to provide verbal feedback
on the formative or summative evaluation process.
